Explanation (?):

The smart card helper service provides help to the smart card service so that it can detect legacy non-plug nad play smart card readers. This just means that this service lets Windows use old smart card readers. It should be noted that this service was removed in XP's Service Pack 2 and the features were moved into the smart card service. You should leave this service set to manual as it only starts when it is needed, but it is safe to disable if you do not have an old smart card reader.
